Hide Name Field & Disable Background

    Environment:
    -Optin Pop-Up w/ Theme 6
    -Autoresponder: Send Optins to Email

    Issue 1: I don’t see where I can hide the Name field.
    Issue 2: I’d like to be able to hide/disable the background and only show the overlay
